ABSTRACT

SUMMARY: Exposure to air pollution and its pollutants has been associated with important effects on human health since the first years of life, thus it has been seen that exposure to tobacco smoke and wood smoke is directly related to cardiovascular and pulmonary diseases, respiratory and cancers. However, exposure to air pollution during fetal development and its effects on brain structure and function during early childhood and adolescence have been little studied. In this review we have analyzed the literature on prenatal exposure to tobacco and wood smoke and its relationship with hypothalamic development and cognition in the first years of life.The molecular, morphological and physiological aspects of the relationship between pre- and postnatal exposure to tobacco and wood smoke with neural developmental, cognitive and behavioral problems during early childhood and adolescence have not yet been fully clarified. The information available in the scientific literature based on antecedents obtained from epidemiological studies has been negatively affected by confounding variables and great methodological challenges that make it impossible to affirm an exact causal relationship with certainty.


RESUMEN: La exposición a la contaminación del aire se ha asociado con importantes efectos en la salud humana desde los primeros años de vida. Estudios han demostrado con certeza que la exposición al humo de tabaco y humo de leña está directamente relacionada con enfermedades cardiovasculares, pulmonares, respiratorias y cánceres. Sin embargo, la exposición a la contaminación del aire durante el desarrollo fetal y sus efectos a posteriori sobre la estructura y función del cerebro durante la primera infancia y la adolescencia son aún desconocidos. En esta revisión analizamos la literatura sobre la exposición prenatal al tabaco y al humo de leña y su relación con el desarrollo hipotalámico y la cognición en los primeros años de vida. Los aspectos moleculares, morfológicos y fisiológicos de la asociación entre la exposición pre y postnatal al humo de tabaco o al humo de leña con problemas del desarrollo neurológico normal, cognitivos y de comportamiento durante la primera infancia y la adolescencia aún no se han aclarado completamente. La información disponible en la literatura científica basada en antecedentes obtenidos de estudios epidemiológicos ha sido afectada negativamente por variables de confusión y grandes desafíos metodológicos que hacen imposible afirmar una relación directa y causal exacta con certeza.

Resumo Objetivo Analisar os sinais e sintomas apresentados por médicos residentes das clínicas cirúrgicas e anestesiologia expostos à fumaça cirúrgica. Métodos Estudo de coorte prospectivo realizado com médicos residentes expostos a fumaça cirúrgica em um hospital universitário. Houve um acompanhamento durante 17 meses dos residentes ingressantes nos anos de 2015 e 2016, que atendiam aos critérios de estar regularmente matriculado na residência de clínica cirúrgica ou anestesiologia e não ser tabagista. O instrumento de coleta de dados foi composto de dados sociodemográficos e acadêmicos e dos sinais e sintomas relacionados com a inalação da fumaça cirúrgica, citados na literatura. A análise de dados ocorreu de forma descritiva e inferencial, por testes estatísticos e medidas de efeito. Resultados A amostra foi composta por 39 residentes, cuja maioria era do sexo masculino (56,4%) e idade abaixo dos 30 anos (74,3%). Prevaleceram residentes da ginecologia e obstetrícia (30,8%), seguidos de cirurgia geral (28,2%) e anestesiologia (20,5%). Ardência na faringe (p=0,030), náusea e vômito (p=0,018) e irritação dos olhos (p=0,050) incidiram ainda no primeiro ano de residência. O risco de desenvolver ardência de faringe foi 7,765 vezes (p=0,019) no sexo feminino em relação ao masculino. Conclusão Os sinais e sintomas analisados incidiram em até 12 meses do início da residência e o risco de apresentar ardência de faringe foi maior no sexo feminino, o que indica a exposição aos riscos da inalação da fumaça cirúrgica e, portanto, a necessidade de adoção de medidas de proteção individuais e coletivas.


Resumen Objetivo analizar las señales y síntomas presentados por médicos residentes de clínica quirúrgica y anestesiología expuestos al humo quirúrgico. Métodos estudio de cohorte prospectivo realizado con médicos residentes expuestos al humo quirúrgico en un hospital universitario. Hubo un seguimiento durante 17 meses de los residentes que ingresaron en 2015 y 2016, que cumplían los criterios de estar regularmente matriculados en la residencia de clínica quirúrgica o anestesiología y no ser fumadores. El instrumento de recolección de datos fue compuesto por datos sociodemográficos y académicos y por señales y síntomas relacionados con la inhalación de humo quirúrgico, citados en la literatura. El análisis de datos se realizó de forma descriptiva e inferencial, por pruebas estadística y medidas de efecto. Resultados la muestra fue compuesta por 39 residentes, cuya mayoría era de sexo masculino (56,4%) y menores de 30 años (74,3%). Prevalecieron residentes de ginecología y obstetricia (30,8%), seguidos de cirugía general (28,2%) y anestesiología (20,5%). Ardor de faringe (p=0,030), náuseas y vómitos (p=0,018) e irritación de ojos (p=0,050) incidieron en el primer año de residencia. El riesgo de desarrollar ardor de faringe fue 7,765 veces (p=0,019) en el sexo femenino con relación al masculino. Conclusión las señales y síntomas analizados incidieron hasta 12 meses desde el inicio de la residencia y el riesgo de presentar ardor de faringe fue mayor en el sexo femenino, lo que indica una exposición a los riesgos de inhalación de humo quirúrgico y, por lo tanto, la necesidad de adoptar medidas de protección individuales y colectivas.


Abstract Objective To analyze the signs and symptoms presented by doctors in surgery and anesthesiology residency programs exposed to surgical smoke. Method Prospective cohort study with resident doctors exposed to surgical smoke in a teaching hospital. There was 17-month follow-up of residents from the years 2015 and 2016, who met the criteria of being regularly enrolled in a surgery or anesthesiology residency and not being a smoker. The data collection instrument was composed of sociodemographic, academic data and the signs and symptoms related to the inhalation of surgical smoke, cited in the literature. The data analysis was descriptive and inferential, by statistical tests and measures of effect. Results The sample consisted of 39 residents, of which most were male (56.4%) and below 30 years old (74.3%). There was a prevalence of gynecology and obstetrics residents (30.8%), followed by general surgery (28.2%) and anesthesiology (20.5%). Burning in the pharynx (p=0.030), nausea and vomiting (p=0.018) and eye irritation (p=0.050) occurred in the first year of residence. The risk of developing burning in the pharynx was 7.765 times greater (p=0.019) in females when compared to males. Conclusion The signs and symptoms analyzed occurred within 12 months of the beginning of the course and the risk of burning in the pharynx was higher in females, which indicates exposure to the risks of inhalation of surgical smoke and, therefore, points to the need for the adoption of individual and collective protection measures.

ABSTRACT Objective: Identify the association between exposure to fine particulate matter and hospitalizations due to respiratory diseases in children up to ten years of age in the city of Cuiabá, Mato Grosso, stratifying the analysis by sex and calculating excess costs. Methods: Ecological study of time series. The dependent variable was daily hospitalizations according to the 10th Revision of the International Classification of Diseases (ICD10): J04.0, J12.0 to J18.9, J20.0 to J21.9 and J45.0 to J45.0. The independent variables were the concentration of fine particulate, estimated by a mathematical model, temperature and relative air humidity, controlled by short and long-term trends. Generalized additive model of Poisson regression was used. Relative risks, proportional attributable risk (PAR) and excess hospitalizations and their respective costs by the population attributable fraction (PAF) were calculated. Results: 1,165 children were hospitalized, 640 males and 525 females. The mean concentration, estimated by the mathematical model, was 15.1±2.9 mcg/m3 for PM2.5. For boys, there was no significant association; for girls a relative risk of up to 1.04 of daily hospitalizations due to respiratory diseases was observed for exposure to PM 2.5 in lags 1, 2 and 6. Increase of 5 µg/m3 in these concentrations increased the percentage of the risk in 18%; with an excess 95 hospital admissions and with excess expenses in the order of US$ 35 thousand. Conclusions: Significant effect in daily hospitalizations due to respiratory diseases related to exposure to fine particulate matter was noted for girls, suggesting the need for stratification by sex in further studies.


RESUMO Objetivo: Identificar associação entre exposição ao material particulado fino e internações por doenças respiratórias em crianças de até dez anos de idade, no município de Cuiabá, MT, estratificando a análise por sexo e calculando excesso de custos. Métodos: Estudo ecológico de séries temporais, sendo a variável dependente o número diário de internações por doenças respiratórias, segundo a 10a Revisão da Classificação Internacional de Doenças (CID): J04.0, J12.0 a J18.9, J20.0 a J21.9 e J45.0 a J45.0. As variáveis independentes foram a concentração do particulado fino, estimada pelo modelo matemático, além de temperatura e umidade relativa do ar, controladas pelas tendências de curta e longa duração. Foram calculados riscos relativos, risco atribuível proporcional (RAP) e excessos de internações e seus respectivos custos pela fração atribuível populacional (FAP). Resultados: Foram internadas 1.165 crianças (640 meninos e 525 meninas). A concentração média estimada do particulado fino foi 15,1±2,9 mcg/m3 para particulado fino (PM2,5). Para meninos, não houve associação significativa. Para meninas, observou-se risco relativo (RR) de até 1,04 para o número diário de internações por doenças respiratórias em associação à exposição ao PM2,5 nos lags 1, 2 e 6. O aumento de 5 mcg/m3 nessas concentrações associou-se ao aumento do risco em 18% e o RAP atribuído à exposição foi de 20% das internações das meninas, com excesso de 95 internações e de gastos da ordem de R$ 105 mil. Conclusões: Houve associação da exposição ao particulado fino e número de internações por doenças respiratórias de meninas, sugerindo a necessidade de estratificação por sexos em estudos posteriores.

ABSTRACT BACKGROUND: Exposure to some air pollutants is associated with cardiovascular diseases. The objective of this study was to quantify the effect of exposure to fine particulate matter in hospitalizations due to ischemic heart disease and the costs to the healthcare system. DESIGN AND SETTING: Time-series ecological study conducted in Taubaté, Brazil. METHODS: Data on hospitalizations due to ischemic heart diseases (ICD I-20 to I-24) in the municipality of Taubaté (SP), Brazil, among adults of both sexes aged 40 years and over, from August 2011 to July 2012, were obtained from DATASUS. Fine particulate matter (PM2.5) concentrations were estimated from a mathematical model. Poisson regression was used in statistical analyses to estimate the relative risks of exposure to PM2.5 for both sexes and after stratification according to sex. The excess of hospitalizations and consequent excess expenditure for the healthcare system were calculated. RESULTS: There were 1040 admissions, among which 382 had ischemic heart diseases (257 males). Themean PM2.5 concentration was 13.2 µg/m3 (SD = 5.6). Significant effects from exposure were noted 4and 5 days after exposure (lag 4 and lag 5) for both sexes and for male sex; for female sex, the effect was 2 days after exposure (lag 2). There were 59 excess hospitalizations for an increase in PM2.5 concentration of 5 µg/m3 and excess expenditure of US$ 150,000 for the National Health System. CONCLUSIONS: An excess of hospital admissions due to ischemic heart disease, with excess expenditure, was identified consequent to PM2.5 exposure.

Abstract Purpose: To evaluate the hyaluronic acid (HA) inflammatory reaction, fibroblasts, fibrosis and duration of effect in the dorsal region of tobacco-exposed rats. Methods: Ten Wistar rats were divided into two groups: tobacco-exposed-group (TEG;n=5) and air-control-group (CG;n=5). The TEG animals were tobacco-exposed twice a day, 30-minutes/session, during 60 days. After this period, all animals received 0.1 mL HA subcutaneous injection in the dorsal area. The volume of HA was measured immediately after HA injection and weekly using a hand-caliper in nine weeks. After this period, all the animals were euthanized, and a specimen of was collected to evaluate inflammatory cells, fibroblasts, and fibrosis by HE. Results: This study showed a higher inflammatory reaction in TEG than CG: inflammatory cell-count (CG: 1.07±0.9; TEG: 8.61±0.36, p<0.001); fibroblast count (CG: 2.92±0.17; TEG: 19.14±0.62, p<0.001), and fibrosis quantification (CG: 2.0; TEG: 3.75, p<0.001). The analysis of the HA volume in nine weeks in the dorsal region did not show a difference between groups (p=0.39). Conclusions: This study suggested that the HA injection in the TEG caused an increase in inflammatory cell count, fibroblast, and fibrosis quantification when compared to the CG. There was no difference in the duration of effect of HA between the groups.

RESUMEN Objetivos Describir e interpretar las metodologías y resultados de los principales estudios relacionados con el monitoreo de contaminación atmosférica en ciclistas. Métodos Se buscaron y analizaron las investigaciones a nivel nacional e internacional de los últimos diez años en las bases de datos Cochrane, Scopus, Embase, Science Direct y Pubmed. La búsqueda se realizó de Agosto a Septiembre del año 2017 usando los siguientes descriptores de búsqueda para MeSH: air pollution, bicycling, environmental exposure, enviromental health, inhalation exposure, environmental pollutants, transportation, public health y toxicology, y para DeCS: contaminación del aire, ciclismo, exposición a riesgos ambientales, contaminantes ambientales, inhalación, transportes, salud pública y toxicología. Resultados Se identificaron diecinueve artículos publicados elegibles. La mayoría de los estudios se realizaron en Europa y Estados Unidos. En Latinoamérica se han reportado cuatro estudios. Otros estudios buscaban comparar la exposición en distintas rutas en bicicleta, comparar trayectos de una misma ruta, y otros determinar la relación distancia vs exposición. De igual manera, variables como el tráfico vehicular, la distancia a las fuentes de emisión y el tipo de ciclorruta, juegan un papel fundamental en la exposición a contaminantes en ciclistas. Conclusiones Se encontraron diversas variables que influyen de forma directa o indirectamente en la exposición a la contaminación del aire de los usuarios de bicicleta, así como algunos factores que pueden reducir la exposición a estos contaminantes.(AU)


ABSTRACT Objectives To describe and interpret the methodologies and results of the main studies related to the monitoring of exposure of cyclists to air pollution. Methods Research and analysis of national and international research of the last ten years in the Cochrane, Scopus, Embase, Science Direct and Pubmed databases. The search was conducted in August and September 2017 using the following search descriptors for MeSH: air pollution, bicycle riding, environmental exposure, environmental health, exposure by inhalation, environmental pollutants, transportation, public health and toxicology. For DeCS: air Pollution, cycling, exposure to environmental risks, environmental pollutants, inhalation, transportation, public health and toxicology. Results Nineteen eligible published articles were identified. Most studies were conducted in Europe and the United States. Four studies have been reported in South America. Other studies seek comparisons on different bicycle routes, compare exposure during variations of the same route, and others determine the distance-exposure relationship. In the same way, variables such as vehicular traffic, distance to the emission sources and the type of bike path, play a fundamental role in the exposure to pollutants in cyclists. Conclusions Several variables were found that influence, directly or indirectly, the cyclists exposure to air pollution, as well as some factors that can reduce this exposure.(AU)

Introducción: la exposición crónica al benceno y tolueno produce alteraciones sobre la médula ósea y el sistema nervioso central, entre otros. En orina, el ácido trans, trans mucónico (t, t-MA) es uno de los biomarcadores de exposición al benceno y el o-cresol (oCre) al tolueno. Objetivo: analizar los resultados de los niveles de t, t-MA y oCre urinarios en una población infanto-juvenil residente en Ciudad Autónoma de Buenos Aires y Conurbano Bonaerense potencialmente expuesta a benceno y tolueno ambiental. Materiales y métodos: se realizó un estudio retrospectivo de los resultados de t, t-MA y oCre urinarios. Las muestras de orina ingresaron al laboratorio con solicitud de t, t-MA (n=1519) y oCre (n=1447) durante el período 2011-2017 (rango etario entre 0 a 19 años). El t, t-MA se cuantificó por UFLC con detector de arreglo de diodos y el oCre por CG con detector de ionización por llama. Resultados: la edad promedio de los pacientes fue de 4,8 años y la mediana 4,6 años.Las concentraciones de t, t-MA urinario fueron: menor de 50 μg/l (44,8%); entre 50-500 μg/l (52,1%) y mayores de 500 μg/l (3,1%). Expresadas por gramo de creatinina: entre 15-163 μg/g creatinina (60,4%) y mayores de 163 μg/g creatinina (39,6%). El límite de cuantificación de t, t-MA fue de 50 μg/l. Las concentraciones de oCre urinario fueron: menor de 0,20 mg/l (97,7%) y entre 0,20-0,50 mg/l (2,3%) y mayor de 0,50 mg/l (0%). Expresadas por gramo de creatinina: menor de 0,30 mg/g creatinina en el 0,8% y mayores de 0,30 mg/g creatinina en el 1,5%. El límite de cuantificación de oCre fue de 0,20 mg/l. Conclusiones: los resultados del trabajo podrían indicar una contaminación ambiental persistente, en especial en el Conurbano Bonaerense, donde debería continuarse el monitoreo de algunas zonas. Pero, por otro lado, es de vital importancia tener en cuenta los factores de confusión, tales como la dieta, la exposición al humo de tabaco ambiental (fumador pasivo) y la tasa de excreción renal que llevarían a una sobre-estimación de los resultados y a una incorrecta toma de decisiones.


Introduction: Chronic exposure to benzene and toluene produces alterations in the bone marrow and the central nervous system, among other effects. In urine, trans, trans muconic acid (t, t-MA) is one of the biomarkers of exposure to benzene and o-cresol (oCre), to toluene. Objective: To analyze the results of urinary t, t-MA and oCre levels in an infant-juvenile population resident in the Autonomous City of Buenos Aires and the Conurbano Bonaerense, potentially exposed to environmental benzene and toluene. Materials and methods: A retrospective study of urinary t, t-MA and oCre results was performed. The urine samples entered the laboratory with the request of t, t-MA (n = 1519) and oCre (n = 1447) during the period 2011- 2017. The age range of the population was between 0 and 19 years. The t, t-MA was quantified by UFLC with diode array detector and the oCre by GC with flame ionization detector. Results: The average age of the patients was 4.8 years and the median age was 4.6 years. The urinary concentrations of t, t-MA were: below 50 μg/l (44.8%); between 50-500 μg/l (52.1%) and above 500 μg/l (3.1%). Expressed per gram of creatinine: between 15-163 μg/g creatinine (60.4%) and greater 163 μg/g creatinine (39.6%). The limit of quantification of t, t-MA was 50 μg/l. The urinary oCre concentrations were: less than 0.20 mg/l (97.7%) and between 0.20-0.50 mg/l (2.3%). Expressed per gram of creatinine: less than 0.30 mg/g creatinine in 0.8% and greater than 0.30 mg/g creatinine in 1.5%. The limit of quantification of oCre was 0.20mg/l. Conclusions: The results of the study could indicate persistent environmental contamination, especially in the Conurbano Bonaerense, where monitoring of some areas should be continued. However, it is of vital importance to take into account the confounding factors, such as diet, exposure to environmental tobacco smoke (passive smoking) and the rate of renal excretion, which would lead to an over-estimation of the results and to incorrect decision-making.

Abstract Purpose: To investigate if the inorganic bovine bone matrix changes the bone formation in rats submitted to inhalation of cigarette smoke. Methods: Twenty Wistar rats were divided into two groups: Cigarette Clot Group (CCG), which in the inhalation chamber received the smoke of 10 cigarettes, 3 times a day, 10 minutes, for 30 days and had the surgical cavity filled by clot; Cigarette Biomaterial Group (CBG), submitted to the same inhalation technique but with the cavity filled by biomaterial. Results: In CCG there was a significant difference of new bone tissue in the analyzed periods (15 and 45 days), and in 15 days, there was 4.8 ± 0.42 of bone formed and 11.73 ± 0.59 (p <0.05) in 45 days. The CBG also showed a significant difference between the periods of 15 to 45 days, being respectively 6.16 ± 0.30 and 11.60 ± 0.61. However, when the groups were compared, within the same analyzed periods, a significant difference was observed only in the period of 15 days, with the new bone percentage being greater in the CBG. Conclusion: The bone matrix acted as an osteoinductive biomaterial, biocompatible and aided in the repair process, mainly in the initial period of recovery.

ABSTRACT

Exposição a poluentes do ar, que costumam ser quantificados por agências ambientais que não estão presentes em todos os estados, pode estar associada a internações por doenças respiratórias de crianças. Foi desenvolvido um estudo ecológico de séries temporais com dados referentes às internações por algumas doenças respiratórias de crianças menores de dez anos de idade, em 2012, na cidade de Cuiabá, Mato Grosso, Brasil. Os níveis médios de material particulado fino (PM2,5) foram estimados por modelo matemático, os dados de temperatura mínima e umidade relativa do ar foram obtidos do Instituto Nacional de Meteorologia, e número de focos de queimadas do Sistema de Informações Ambientais. A abordagem estatística utilizou o modelo aditivo generalizado da regressão de Poisson com defasagens de 0 a 7 dias. Foram estimados os custos financeiros e aumentos do número de internações decorrentes de elevações de PM2,5. Foram 565 internações (média de 1,54/dia; DP = 1,52) e concentração de PM2,5 de 15,7µg/m3 (DP = 3,2). Foram encontradas associações entre exposição e internações no segundo semestre, nos lags 2 e 3, e quando analisado o ano todo, no lag 2. Uma elevação de 5µg/m3 do PM2,5 implicou o aumento de 89 internações e custos acima dos R$ 95 mil para o Sistema Único de Saúde. Dados estimados por modelo matemático podem ser utilizados em locais onde não há monitoramento de poluentes.


La exposición a contaminantes del aire, que suelen ser cuantificados por agencias ambientales que no están presentes en todos los estados, puede estar asociada a internamientos por enfermedades respiratorias de niños. Se desarrolló un estudio ecológico de series temporales con datos referentes a los internamientos por algunas enfermedades respiratorias de niños menores de 10 años de edad, en 2012, en la ciudad de Cuiabá, Mato Grosso, Brasil. Los niveles medios de material particulado fino (PM2,5) se estimaron mediante un modelo matemático, los datos de temperatura mínima y humedad relativa del aire se obtuvieron del Instituto Nacional de Meteorología, y el número de focos de incendios del Sistema de Información Ambiental. El enfoque estadístico usó el modelo aditivo generalizado de la regresión de Poisson con desfases de 0 a 7 días. Se estimaron los costes financieros y aumentos del número de internamientos derivados de elevaciones de PM2,5. Fueron 565 internamientos (media de 1,54/día; DE = 1,52) y concentración de PM2,5 de 15,7µg/m3 (DE = 3,2). Se encontraron asociaciones entre exposición e internamientos en el segundo semestre, en los lags 2 y 3, y cuando se analizó todo el año, en el lag 2. Una elevación de 5µg/m3 del PM2,5 implicó el aumento de 89 internamientos y costes por encima de los BRL 95 mil para el Sistema Único de Salud. Los datos estimados por el modelo matemático pueden ser utilizados en lugares, donde no existe un monitoreo de contaminantes.


Exposure to air pollutants, usually measured by environmental agencies that are not present in all states, may be associated with respiratory admissions in children. An ecological time series study was conducted with data on hospitalizations due to selected respiratory diseases in children under 10 years of age in 2012 in the city of Cuiabá, Mato Grosso State, Brazil. Mean levels of fine particulate matter (PM2.5) were estimated with a mathematical model, data on low temperatures and relative humidity were obtained from the Brazilian National Institute of Meteorology, and the numbers of brush burnings were obtained from the Environmental Information System. The statistical approach used the Poisson regression generalized additive model with lags of 0 to 7 days. The financial costs and increases in hospitalizations due to increments in PM2.5 were estimated. There were 565 hospitalizations (mean 1.54 admissions/day; SD = 1.52), and mean PM2.5 concentration was 15.7µg/m3 (SD = 3.2). Associations were observed between exposure and hospitalizations in the second semester at lags 2 and 3, and at lag 2 when the entire year was analyzed. An increment of 5µg/m3 in PM2.5 was associated with an increase of 89 hospitalizations and costs exceeding BRL 95,000 (≈ USD 38,000) for the Brazilian Unified National Health System. Data estimated by mathematical models can be used in locations where pollutants are not monitored.

OBJECTIVES: To evaluate the influence of secondhand cigarette smoke exposure on longitudinal growth of the tibia of growing rats and some parameters of bone quality. METHODS: Forty female rats were randomly divided into four groups: control: rats were sham exposed; 30 days: rats were exposed to tobacco smoke for 30 days; 45 days: rats were exposed to tobacco smoke for 45 days; and 60 days: rats were exposed to tobacco smoke for 60 days. Blood samples were collected to evaluate the levels of cotinine and alkaline phosphatase. Both tibias were dissected and weighed; the lengths were measured, and the bones were then stored in a freezer for analysis of bone mineral content and mechanical resistance (maximal load and stiffness). RESULTS: Exposure of rats to tobacco smoke significantly compromised bone health, suggesting that the harmful effects may be time dependent. Harmful effects on bone growth were detected and were more pronounced at 60-day follow-ups with a 41.8% reduction in alkaline phosphatase levels (p<0.01) and a decrease of 11.25% in tibia length (p<0.001). Furthermore, a 41.5% decrease in bone mineral density was observed (p<0.001), leading to a 42.8% reduction in maximum strength (p<0.001) and a 56.7% reduction in stiffness (p<0.001). CONCLUSION: Second hand cigarette smoke exposure in rats affected bones that were weaker, deforming them and making them osteopenic. Additionally, the long bone was shorter, suggesting interference with growth. Such events seem to be related to time of exposure.

ABSTRACT Objective: To evaluate the effects of exposure to emissions from sugar cane burning on inflammatory mechanisms in tissues of the trachea and lung parenchyma in Wistar rats after different periods of exposure. Methods: This was an experimental open randomized study. The animals were divided into four groups: a control group (CG) underwent standard laboratory conditions, and three experimental groups were exposed to emissions from sugar cane burning over different periods of time, in days-1 (EG1), 7 (EG7), and 21 (EG21). After euthanasia with 200 mg/kg of ketamine/xylazine, fragments of trachea and lung were collected and fixed in 10% formalin. Histological analyses were performed with H&E and picrosirius red staining. Results: No inflammatory infiltrates were found in the tissues of CG rats. The histological examination of tissues of the trachea and lung parenchyma revealed that the inflammatory process was significantly more intense in EG7 than in the CG (p < 0.05 and p < 0.01, respectively). In comparison with the CG and EG1, angiogenesis in the lung parenchyma and collagen deposition in tracheal tissues were significantly greater only in EG21 (p < 0.001 and p < 0.01, respectively). Conclusions: In this sample, emissions from sugar cane burning induced acute focal and diffuse inflammation in the lamina propria of tracheal tissues, with no loss of ciliated epithelial tissue. In the lung parenchyma of the animals in the experimental groups, there was interstitial and alveolar edema, together with polymorphonuclear cell infiltrates.


RESUMO Objetivo: Avaliar os efeitos da exposição à fumaça da queima da cana-de-açúcar sobre mecanismos inflamatórios em tecidos de traqueia e de parênquima pulmonar de ratos Wistar após diferentes períodos de exposição. Métodos: Estudo experimental, randomizado, não cego. Os animais foram divididos em quatro grupos: controle (GC), sob condições padrão de laboratório e os demais expostos à fumaça da queima da cana-de-açúcar por diferentes períodos: em 1 (GE1), 7 (GE7) e 21 (GE21) dias. Após a eutanásia com 200 mg/kg de ketamina/xilazina, foram coletados fragmentos de traqueia e pulmão e fixadas em formol 10%. Análises histológicas foram realizadas com coloração com H&E e picrosírius. Resultados: Não houve infiltrado inflamatório nos tecidos no GC. O processo inflamatório na análise histológica de tecidos de traqueia e de parênquima pulmonar foi significativamente mais intenso no GE7 quando comparado ao GC (p < 0,05 e p < 0,01, respectivamente). Em comparação com os grupos GC e GE1, apenas no GE21 foi observada angiogênese significativa no parênquima pulmonar e aumento significativo de depósitos de colágeno em tecido de traqueia (p < 0,001 e p < 0,01, respectivamente). Conclusões: Nesta amostra, a fumaça da queima de cana-de-açúcar induziu processo inflamatório focal, difuso e agudo em tecidos de traqueia na lâmina própria, sem perda do tecido epitelial ciliado. Houve presença de edemas intersticiais e alveolares e infiltrados de células polimorfonucleares no parênquima pulmonar nos animais dos grupos experimentais.

ABSTRACT OBJECTIVE To evaluate the effects of acute exposure to air pollutants (NO2 and PM10) on hospitalization of adults and older people with cardiovascular diseases in Western São Paulo. METHODS Daily cardiovascular-related hospitalization data (CID10 – I00 to I99) were acquired by the Department of Informatics of the Brazilian Unified Health System (DATASUS) from January 2009 to December 2012. Daily levels of NO2 and PM10 and weather data were obtained from Companhia Ambiental do Estado de São Paulo (CETESB – São Paulo State Environmental Agency). To estimate the effects of air pollutants exposure on hospital admissions, generalized linear Poisson regression models were used. RESULTS During the study period, 6,363 hospitalizations were analysed. On the day of NO2 exposure, an increase of 1.12% (95%CI 0.05–2.20) was observed in the interquartile range along with an increase in hospital admissions. For PM10, a pattern of similar effect was observed; however, results were not statistically significant. CONCLUSIONS Even though with values within established limits, NO2 is an important short-term risk factor for cardiovascular morbidity.

ABSTRACT OBJECTIVE The objective of this study has been to analyze whether fine particulate matter (PM2.5), as well as its synergistic effect with maximum temperature, humidity, and seasons, is associated with morbidity and mortality from cardiovascular diseases. METHODS This is an ecological study of time series. We have used as outcomes the daily death and hospitalization records of adults aged 45 years and over from 2009 to 2011 of the municipalities of Cuiabá and Várzea Grande, State of Mato Grosso, Brazil. We have used Poisson regression using generalized additive models, assuming a significance level of 5%. The model has been controlled for temporal trend, seasonality, average temperature, humidity, and season effects. Daily concentrations of PM2.5 (particulate material with aerodynamic diameter less than 2.5 micrometers) have been obtained by converting the values of optical aerosol thickness. Maximum temperature, humidity, and seasons have been separately included in the model as dummy variables for the analysis of the synergistic effect of PM2.5 with morbidity and mortality from cardiovascular disease. We have calculated the percentage increase of relative risk (%RR) of deaths and hospitalizations for the linear increase of 10 μg/m3 of PM2.5. RESULTS Between 2009 and 2011, the increase in PM2.5 was associated with a %RR 2.28 (95%CI 0.53-4.06) for hospitalizations on the same day of exposure and RR% 3.57 (95%CI 0.82-6.38) for deaths with a lag of three days. On hot days, %RR 4.90 (95%CI -0.61-9.38) was observed for deaths. No modification of the effect of PM2.5 was observed for maximum temperature in relation to hospitalizations. On days with low humidity, %RR was 5.35 (95%CI -0.20-11.22) for deaths and 2.71 (95%CI -0.39-5.92) for hospitalizations. In the dry season, %RR was 2.35 (95%CI 0.59-4.15) for hospitalizations and 3.43 (95%CI 0.58-6.35) for deaths. CONCLUSIONS The PM2.5 is associated with morbidity and mortality from cardiovascular diseases and its effects may be potentiated by heat and low humidity and during the dry season.


RESUMO OBJETIVO Analisar se o material particulado fino (PM2,5), bem como seu efeito sinérgico com a temperatura máxima, a umidade e as estações do ano estão associados à morbimortalidade por doenças cardiovasculares. MÉTODOS Estudo ecológico de séries temporais. Foram utilizados como desfechos os registros diários de óbito e internação em adultos com 45 anos ou mais de idade para os anos de 2009 a 2011 nos municípios de Cuiabá e Várzea Grande, Mato Grosso, Brasil. Utilizou-se regressão de Poisson via modelos aditivos generalizados, assumindo-se um nível de significância de 5%. O modelo foi controlado para tendência temporal, sazonalidade, temperatura média, umidade e efeitos de calendário. Concentrações diárias de PM2,5 (material particulado com diâmetro aerodinâmico inferior a 2,5 micrômetros) foram obtidas por meio da conversão dos valores da espessura ótica de aerossóis. Temperatura máxima, umidade e estações do ano foram incluídas separadamente ao modelo como variáveis indicadoras para análise do efeito sinérgico do PM2,5 com a morbimortalidade por doenças cardiovasculares. Foi calculado o aumento percentual de risco relativo (%RR) dos óbitos e internações para o aumento linear de 10 μg/m3 de PM2,5. RESULTADOS Entre 2009 e 2011, o aumento de PM2,5 foi associado a um %RR 2,28 (IC95% 0,53-4,06) para internações no mesmo dia de exposição e %RR 3,57 (IC95% 0,82-6,38) para óbitos com uma defasagem de três dias. Em dias quentes, observou-se %RR 4,90 (IC95% -0,61-9,38) para óbitos. Não foi observada modificação de efeito do PM2,5 pela temperatura máxima relacionada as internações. Em dias com baixa umidade, o %RR foi 5,35 (IC95% -0,20-11,22) para óbitos e 2,71 (IC95% -0,39-5,92) para internações. Na estação seca, o %RR foi 2,35 (IC95% 0,59-4,15) para internações e 3,43 (IC95% 0,58-6,35) para óbitos. CONCLUSÕES O PM2,5 está associado à morbimortalidade por doenças cardiovasculares e seus efeitos podem ser potencializados pelo calor, pela baixa umidade e durante a estação seca.

ABSTRACT Objective: To evaluate the effects of passive inhalation of cigarette smoke on the respiratory system of guinea pigs. Methods: Male guinea pigs were divided into two groups: control and passive smoking, the latter being exposed to the smoke of ten cigarettes for 20 min in the morning, afternoon and evening (30 cigarettes/day) for five days. After that period, inflammatory parameters were studied by quantifying mesenteric mast cell degranulation, as well as oxidative stress, in BAL fluid. In addition, we determined MIP, MEP, and mucociliary transport (in vivo), as well as tracheal contractility response (in vitro). Results: In comparison with the control group, the passive smoking group showed a significant increase in mast cell degranulation (19.75 ± 3.77% vs. 42.53 ± 0.42%; p < 0.001) and in the levels of reduced glutathione (293.9 ± 19.21 vs. 723.7 ± 67.43 nM/g of tissue; p < 0.05); as well as a significant reduction in mucociliary clearance (p < 0.05), which caused significant changes in pulmonary function (in MIP and MEP; p < 0.05 for both) and airway hyperreactivity. Conclusions: Passive inhalation of cigarette smoke caused significant increases in mast cell degranulation and oxidative stress. This inflammatory process seems to influence the decrease in mucociliary transport and to cause changes in pulmonary function, leading to tracheal hyperreactivity.


RESUMO Objetivo: Avaliar os efeitos da inalação passiva da fumaça de cigarro no sistema respiratório de cobaias. Métodos: Foram utilizadas cobaias machos, divididas em dois grupos: controle e tabagismo passivo, esse último exposto à fumaça de dez cigarros por 20 min pela manhã, tarde e noite (30 cigarros/dia) por 5 dias. Após esse período, parâmetros inflamatórios foram estudados através da contagem de degranulação de mastócitos no mesentério e de estresse oxidativo a partir do LBA. Adicionalmente, foram verificadas PImáx, PEmáx, transporte mucociliar (in vivo) e contratilidade traqueal (in vitro). Resultados: Na comparação com o grupo controle, o grupo tabagismo passivo apresentou um aumento significativo na degranulação de mastócitos (19,75 ± 3,77% vs. 42,53 ± 0,42%; p < 0,001), nos níveis de glutationa reduzida (293,9 ± 19,21 vs. 723,7 ± 67,43 nM/g de tecido; p < 0,05) e uma redução significativa no transporte mucociliar (p < 0,05), provocando alterações significativas na função pulmonar, tanto na PImáx como na PEmáx (p < 0,05 para ambas), e hiper-reatividade nas vias aéreas. Conclusões: A inalação passiva da fumaça de cigarro ocasionou aumentos significativos na degranulação de mastócitos e no estresse oxidativo. Esse processo inflamatório parece ter influenciado a diminuição do transporte mucociliar e causado alterações na função pulmonar, proporcionando um quadro de hiper-reatividade traqueal.

Abstract Background: There is evidence of the effects of air pollution on hospital admissions due to cardiovascular diseases, including myocardial infarction. Objective: To estimate the association between exposure to air pollutants and hospital admissions due to myocardial infarction according to gender, between January 1st 2012 and December 31st 2013, in São Jose dos Campos-SP. Methods: An ecological time series study was carried out with daily data of admissions due to AMI, pollutants CO, O3, PM10, SO2, and NO2, according to gender. We used the Poisson regression generalized linear model to estimate the relative risks of hospital admissions with lags of 0-5 days, adjusted for temperature, humidity, seasonality and days of the week. Results: There were 1837 admissions for ischemic heart diseases, with 636 women and 1201 men. For females, the risks were significant for CO in lag 0 (RR = 1,09), lag1 (RR = 1,08) and lag 5 (RR = 1,10) and SO2 in lag 0 (RR = 1,10) and 3 (RR = 1,09). For men there was significance of the CO in, lag 3 and lag 5 (RR = 1,05). There was significance, regardless of gender, for CO at lag 1 (RR = 1,05) and lag 5 (RR = 1,07) and lag 0 for SO2 (RR = 1,06). Conclusion: The data presented show the important role of CO and SO2 in the genesis of myocardial infarction admissions, and responses to pollutant exposure are different if analyzed by gender and together - hence the importance of a stratified analyses.


Resumo Fundamento: Existem evidências sobre os efeitos da poluição do ar nas internações por doenças cardiovasculares, entre elas o infarto do miocárdio. Objetivo: Estimar a associação entre exposição a poluentes do ar e internações por infarto segundo gêneros, entre 01 de Janeiro de 2012 e 31 de Dezembro de 2013, em São José dos Campos - SP. Métodos: Estudo ecológico de série temporal com dados diários de internações por IAM dos poluentes CO, O3, PM10, SO2, NO2, segundo gêneros. Utilizou-se modelo linear generalizado da Regressão de Poisson para estimar os riscos relativos para internações com defasagens de 0 a 5 dias, ajustados por temperatura, umidade, sazonalidade e dias da semana. Resultados: Foram 1837 internações por doenças isquêmicas do coração, sendo 636 mulheres e 1201 homens. Para o gênero feminino, os riscos foram significativos para o CO nos lag 0 (RR = 1,09), lag1 (RR = 1,08) e lag 5 (RR = 1,10) e para o SO2 no lag 0 (RR = 1,10) e 3 (RR = 1,09). Para o gênero masculino houve significância para o CO no lag 3 e lag 5 (RR = 1,05). Sem distinção de gênero houve significância para o CO no lag 1 (RR = 1,05) e lag 5 (RR = 1,07) e no lag 0 para o SO2 (RR = 1,06). Conclusão: Os dados apresentados mostram o importante papel do CO e do SO2 na gênese das internações por infarto e que as respostas à exposição aos poluentes são diferentes se analisadas por sexo e em conjunto, daí a importância de se estratificarem as análises.

ABSTRACT

Liquefied hydrocarbon gas, such as propane is considered safe. However there are reports that voluntary exposure to liquefied gas at least could originate hallucinatory states. We report a 20 years old woman who was found in a coma with extensor muscle hypertonia, brisk tendon reflexes and extensor plantar (Babinski) responses after being exposed to propane gas. The brain magnetic resonance imaging (MRI) showed lesions in both hippocampi and white matter in the oval center. The patient had a normal oxygen saturation of 98%, a carboxyhemoglobin of 1.6% and a metabolic acidosis with a pH of 7.01 with an anion gap of 16 mEq/l. This pattern suggested that it was a consequence of the intermediary metabolism of isopropanol. The recovery of the patient was slow and four months later she still had lesions on MRI and limitations in her cognitive sphere, memory and executive functions. Thus, liquefied gas exposure can cause a toxic encephalopathy with a persistent damage of the central nervous system.

ABSTRACT CONTEXT AND OBJECTIVE: Exposure to air pollutants is one of the factors responsible for hospitalizations due to respiratory diseases. The objective here was to estimate the effect of exposure to particulate matter (such as PM2.5) on hospitalizations due to certain respiratory diseases among residents in Volta Redonda (RJ). DESIGN AND SETTING: Ecological time series study using data from Volta Redonda (RJ). METHODS: Data on hospital admissions among residents of Volta Redonda (RJ), between January 1, 2012, and December 31, 2012, due to pneumonia, acute bronchitis, bronchiolitis and asthma, were analyzed. Daily data on PM2.5 concentrations were estimated through the CCATT-BRAMS model. The generalized additive Poisson regression model was used, taking the daily number of hospitalizations to be the dependent variable and the PM2.5 concentration to be the independent variable, with adjustment for temperature, relative humidity, seasonality and day of the week, and using lags of zero to seven days. Excess hospitalization and its cost were calculated in accordance with increases in PM2.5 concentration of 5 µg/m3. RESULTS: There were 752 hospitalizations in 2012; the average concentration of PM2.5 was 17.2 µg/m3; the effects of exposure were significant at lag 2 (RR = 1.017), lag 5 (RR = 1.022) and lag 7 (RR = 1,020). A decrease in PM2.5 concentration of 5 µg/m3 could reduce admissions by up to 76 cases, with a decrease in spending of R$ 84,000 a year. CONCLUSION: The findings from this study provide support for implementing public health policies in this municipality, which is an important steelmaking center.


RESUMO CONTEXTO E OBJETIVO: A exposição aos poluentes do ar é um dos fatores responsáveis por internações por doenças respiratórias; o objetivo foi estimar o efeito da exposição a material particulado (como PM2,5) sobre as hospitalizações devido a certas doenças respiratórias em moradores de Volta Redonda (RJ). TIPO DE ESTUDO E LOCAL: Estudo ecológico de séries temporais utilizando dados de Volta Redonda (RJ). MÉTODOS: Foram analisados dados de internação hospitalar, entre 1 de janeiro de 2012 e 31 de dezembro de 2012, devida a pneumonia, bronquite aguda, bronquiolite e asma, em residentes em Volta Redonda (RJ), e dados diários das concentrações de PM2,5 obtidos a partir do modelo CCATT-BRAMS. Utilizou-se o modelo aditivo generalizado de regressão de Poisson, sendo o número diário de internações a variável dependente e a concentração PM2,5 a variável independente, ajustado para temperatura, umidade relativa, sazonalidade e dia da semana e defasagens entre zero e sete dias. Excesso de internação hospitalar e seu custo, de acordo com o aumento de 5 ug/m3 na concentração de PM2,5, foram calculados. RESULTADOS: Ocorreram 752 internações em 2012; a concentração média de PM2,5 foi de 17,2 ug/m3; os efeitos da exposição foram significativos em lag 2 (RR = 1,017), lag 5 (RR = 1,022) e lag 7 (RR = 1,020). A redução em 5 ug/m3 na concentração de PM2,5 poderia reduzir até 76 casos as internações com diminuição nos gastos de R$ 84 mil/ano. CONCLUSÃO: Os achados deste estudo fornecem subsídios para implantar políticas públicas de saúde neste município, que é importante polo siderúrgico.

Summary Objective: The aim of this study was to estimate the association between expo sure to air pollutants and hospitalization for pneumonia among children in a medium-sized city located in the sugar cane plantation region of São Paulo State. Methods: An ecological time-series study was conducted with daily data of hospi talization for pneumonia including children aged 10 years or younger living in Ar araquara, state of São Paulo, from January 1st, 2010, to November 30th, 2012. To es timate the association between hospitalization due to pneumonia and particulate pollutants with aerodynamic diameter less than 10 µm, nitrogen dioxide and ozone, relative risks for hospitalization according to a generalized additive model of Pois son regression, with Lags of up to five days, were calculated. A percentage increase (PI) was obtained for relative risk (IRR - increase on relative risk) of hospitalization at each 10 µg/m3 increment in each air pollutants adjusted for the remaining. Results: A total of 234 hospitalizations were recorded during these three years. There was a strong association between hospitalization and PM10 and NO2. The PI in relative risk was 15% to PM10 in Lag 0 and 7% points in Lag 1 for NO2. Conclusion: There was evidence of the action of air pollutants on hospitaliza tion for pneumonia in a medium-sized city located in a region affected by air pollution from sugarcane burning and the data presented here provide subsi dies for the implementation of public policies aiming to decrease this risk.


Resumo Objetivo: estimar a associação entre exposição a poluentes atmosféricos e internações por pneumonia em crianças de uma cidade de médio porte em região de plantio de cana-de-açúcar no Estado de São Paulo. Métodos: estudo ecológico de séries temporais com dados de internação por pneumonia em crianças com até 10 anos de idade residentes em Araraquara, SP, de 1 de janeiro de 2010 a 30 de novembro de 2012. Para estimar a associação entre internações por pneumonia e material particulado (MP) com diâmetro aerodinâmico inferior a 10 µm, dióxido de nitrogênio e ozônio, foram calculados os riscos relativos para internação segundo modelo aditivo generalizado de regressão de Poisson, com defasagens de até cinco dias. Foi obtido o aumento percentual (ppt) no risco relativo (ARR - aumento no risco relativo) para internação segundo aumento de 10 µg/m3 para cada poluente ajustado para os demais. Resultados: registraram-se 234 internações nos três anos de estudo. Houve forte associação entre as internações e o MP10 e o NO2. O ARR foi de 15 ppt para MP10 no mesmo dia da exposição e de 7 ppt no primeiro dia após a exposição ao NO2. Conclusão: houve evidências da ação de poluentes do ar sobre internações por pneumonia em uma cidade de médio porte em região afetada por poluição do ar decorrente de queima da palha da cana-de-açúcar. Os dados apresentados fornecem subsídios para implantação de políticas públicas visando à diminuição desses riscos.

ABSTRACT

INTRODUCCIÓN: la inhalación sostenida de polvos inorgánicos en el ambiente laboral puede originar diversas enfermedades respiratorias, conocidas como enfermedades pulmonares de origen ocupacional. OBJETIVO: determinar la asociación entre la exposición prolongada al polvo de mineral laterítico y la ocurrencia de enfermedades respiratorias en los trabajadores expuestos. MÉTODOS: se realizó un estudio observacional, analítico, de cohorte, prospectivo durante los años 2003 al 2012 en una empresa productora de níquel y cobalto, en el municipio Moa. RESULTADOS: predominó el sexo masculino con 91,73 % y el grupo etario más afectado fue el de 45-49 años. De los trabajadores expuestos, 44 presentaron alguna enfermedad respiratoria, con un riesgo relativo de 5,59 y fracción atribuible de 0,8333. La enfermedad pulmonar obstructiva crónica, la neumoconiosis y la infección respiratoria por micobacterias atípicas, se presentaron en el 61,36 %, 22,72 % y 13,63 % de los pacientes, respectivamente. CONCLUSIÓN: se halló una asociación estadísticamente significativa entre la exposición al polvo de mineral laterítico y la ocurrencia de enfermedades respiratorias en los trabajadores expuestos.


INTRODUCTION: sustained inhalation of inorganic dust at the workplace can cause various respiratory diseases, known as lung disease of occupational origin. OBJECTIVE: determine the association between prolonged exposure to dust from lateritic ore and the occurrence of respiratory disease in exposed workers. METHODS: an observational, analytical, cohort study was performed prospectively from 2003 to 2012 in a nickel and cobalt factory, in Moa municipality. RESULTS: male prevailed (91.73 %) and the most affected age group was 45-49 years. 44 out of the exposed workers had respiratory disease, with a relative risk of 5.59 and 0.8333 attributable fractions. Chronic obstructive pulmonary disease, pneumoconiosis and respiratory atypical mycobacterial infection, occurred in 61.36 %, 22.72 % and 13.63 % of patients, respectively. CONCLUSIONS: a statistically significant association between exposure to dust from lateritic ore and the occurrence of respiratory disease in exposed workers was found.
